They need to leapfrog two of Columbus, Toronto, and Montreal. Columbus has 50 points and plays DC, Toronto and Montreal have 49 and 48 points and play each other, and the Revs have 47 points and play NYCFC. So its possible, just unlikely. Simplest scenario Revs win, Columbus loses, and there's a winner in TOR/MON match.
If they make it to the second round they'll have a home playoff match because then its a home-and-away series.
I may have the math wrong, but I think that if NE wins and Columbus loses on the final day, that guarantees the Revs a home game no matter what happens between Toronto and Montreal. That would leave NYRB in first, DC in second, and NE and CLB even at 50 points -- with the first tiebreaker (wins) also even and the second tiebreaker (goal difference) going to the Revs.
So against that backdrop, if Toronto wins, they get the third seed with 52, Revs get the fourth seed with 50 and the tiebreaker, Columbus gets the fifth seed with 50, and MTL gets the sixth seed with 48. If Montreal wins, flip them (51 pts) and Toronto (49 pts), but the Revs stay fourth and still host the play-in. And then if Toronto and Montreal draw, it'll be Toronto as a 3 seed with 50 and the tiebreaker over NE and CLB (most wins), Revs as a 4 seed with 50 and the tiebreaker (GD) over CLB, Columbus as the 5 seed with 50, and Montreal as the 6 seed with 49.
Anything but a Revs win and a Columbus loss and I believe NE has to go on the road for the play-in -- that's assuming, of course, that Orlando doesn't beat Philly by a boatload and NE doesn't lose to NYC by a boatload, leaving NE out of the playoffs entirely. Orlando 4-0 and NYC 4-0 would do the trick. Unlikely, but not completely implausible.
