Arizona did it in a playoff game I believe back in 2010. I'll try finding it as well. But I know Arizona tried the free kick before half time.

The rules allow the ref to do practically whatever he wants if one team commits a "palpably unfair act".

I'm surprised that the league hasn't suspended BB for not calling the time out - it was an obvious situation for a time out and the Seahawks were justified in expecting BB to call it there. His decision not to do so put the Seahawks at an obvious competitive disadvantage in a way that is a clear violation of the competitive integrity policy. (We can see this from the obvious dropoff in Seattle play calling effectiveness and Russell Wilson's performance following the non-timeout.) In the future, league officials need to have the discretion to call a team's timeout in obvious time out situations where the team doesn't call it themselves.

First time I heard about it was the Mac Percival kick in '68 listed in the Wikipedia entry above.    Didn;t realize that there's only been one good one since then.
