I can successfully connection to my media server from within my home network by using the following settings

PC Name: 192.168.1.12
Username: MediaServer\abcuser
Password: abcpassword

Despite having forwarded my ports and taken down my firewall, I can't figure out how to connect from outside my network. I can't figure out how to use my external IP, etc in the settings. Anyone have some guidance?

You need to setup a NAT / inbound Port forwarding. It's not a great idea from a security standpoint though.
 
What type of wireless router / modem do you have?

Fios router. I've forwarded TCP port 3389 to computer 192.168.1.12. Still not working.

Just to start at the beginning (you may already be doing this, I just wasn't totally clear from your post) - to connect from outside, your PC name shouldn't be the "192.xxxxx" #, it should be whatever your actual external IP address is (google what is my ip). On mine (for example) the PC name is "76.167.xxx.xx", and then on my router I set the remote desktop port forwards to 192.168.1.101 (my desktop). The port forwarding handles sending that connection from your router to the computer itself...
 
If you're already doing this (apologies if you already have this), then my guess is that your router is somehow defaulting to block that port... I'm not actually sure how to change that, though probably there's some way

Yes. That's what I have been doing.

By using the external IP, how does the external PC know which PC to connect to on the network? Is the domain (MediaServer) all it needs?

The port forward at the router takes care of that. The external PC talks to the router, and the router forwards the information to the IP you set up.
