iPhone audio doesn't work on calls only

When making or receiving a phone call on iPhone5, I can't hear any audio (out of ear speaker or when on speakerphone mode), and the person on the other end can't hear me.  However, the phone rings on an incoming call, plays music fine so don't think it's a hardware problem?  (Is there any way to refresh/reinstall the Phone app?).
 
I've done the "hold down home key and power key" restart, that didn't fix anything.  Any thoughts?

Dumb question: Are you perhaps connected to a bluetooth headset that you're not wearing?  (turn off bluetooth support to ensure that's not the case).

Thanks - I checked, bluetooth is off.

If you plug in headphones, can you hear during the call?  If so, you might need to clean out the headphone jack thoroughly.

Yeah, I'd suggest cleaning out the headphone jack. It sounds like the phone thinks you have a headset plugged in. This happened to me briefly with my original iPhone. Cleaning out the headphone jack with a trimmed Q-tip solved it immediately.

Sometimes you get dirt and stuff packed down at the bottom, too, and have to go at it with a paper clip first to loosen it up before clearing it out.

Okay i had high hopes it was a gunked up audio jack but i tried cleaning it out and no luck.  
 
I think it's a software issue, because I've since been using Skype to make phone calls and the phone audio works fine on Skype calls - i can hear out of the normal ear speaker or the speaker phone.  When i try making/receiving a call on the phone app, the screen gets freez-y - like when i hit the screen to toggle between ear/speaker or try to change the volume, etc., it's very laggy and freezes up.  
 
I really don't want to re-install to factory condition and all that that entails - is there any way to selectively reinstall a native app?

Restoring really isn't a big deal at all if you have an iCloud or iTunes backup. It literally takes like 30 minutes and your phone will be exactly as it was.

Reset didn't work - going to try to bring to Apple store.
